Contest rules

1. This contest is open to any organization that has purchased and owns a RealCare®  Baby Infant Simulator, RealCare® Baby II or RealCare® Baby II-plus Infant Simulator, BeginnerCare™ Baby Infant Simulator or a ySTART™ PreventPak™ Addiction Simulator.  Individuals who complete the survey and enter the contest, are entering the contest only on behalf of the organizations that have purchased and own any one of the simulators listed in the first sentence.  No individual can enter the contest on his or her own behalf.  Each person completing the survey must be, and asserts that he or she is, 18 years of age, or older. 
   
   
2. Each person entering the contest on behalf of an organization that has purchased and owns any of the simulators mentioned in section 1, above, may enter only once.  Duplicate entries from an email address will void all entries from that email address. Therefore, we request that no individual complete the survey more than once. 
   
   
3. More than one person from an organization may enter, but each entrant must have experience running a program that utilizes any of the Realityworks simulators listed in section 1.
   
   
4. Only one (1) prize will be awarded from this contest.
   
   
5. The prize awarded will be one $200 Product Voucher good toward the purchase of Realityworks products. Realityworks, Inc. reserves the right to substitute a prize of equal or greater value. 
   
   
6. All entries must be received via email no later than Midnight, Central Time in the United States, on August 1, 2008 for entry into the contest.  Entries must be submitted via email to newsletter@realityworks.com.
   
   
7. All entries must include the name of the person completing the survey, the name of the organization the entry is submitted for, the organization’s mailing address and telephone number, and includes the following acknowledgement – that the person completed the survey on behalf of the named organization, that any prize awarded due to his or her completion of the survey will be awarded to, and belong to, the named organization the entry was submitted for, and not to the individual who completes the survey.  Failure to include any of this information with the completed survey, and/or failure to agree to this acknowledgement, will disqualify the survey response from entry into the contest. 
   
   
8. The name of the winning organization will be announced in the Realityworks Star e-newsletter within 3 months following the date that the prize is awarded.
   
   
9. The contest is void where prohibited or regulated.
   
   
10. The contest is open to organizations in the 50 states of the United States, and in the District of Columbia.
